Client Rights & Grievance Policy — Acknowledgment

You are the expert on your own life. These are your rights while you're working with Heart of Care.

As a participant with Heart of Care Full Spectrum CTS, you have the right to: 1. Be treated with dignity, respect, and cultural humility — regardless of race, ethnicity, faith, sex, gender identity, sexual orientation, disability, housing status, income, legal history, or diagnosis. 2. Voluntary participation — you can refuse any service, ask questions, get a second opinion, or leave at any time without losing access to future support. 3. Privacy and confidentiality — subject only to the limits described in our Notice of Privacy Practices and required by law (mandated reporting of child/elder abuse and imminent danger to self or others). 4. Informed consent — a plain-language explanation of anything we do, before we do it. 5. Trauma-informed care — a right to feel safe, to move at your own pace, and to have your "no" honored. 6. Access to your own record — you may see, request a copy of, and ask us to correct information about you. 7. Be free from retaliation for filing a complaint, refusing services, or exercising any of these rights. Grievance process — if you feel any of these rights have been violated, you may: • Speak directly with Briana F. Harden, CPS-MH — the Honey Badger behind Heart of Care. • Email hello@heart-of-care.life or write to us at our published mailing address. • Ask for a written response within 10 business days. • If you are not satisfied, escalate to the Georgia Department of Behavioral Health and Developmental Disabilities Office of Constituent Services, or to your funding source. No one at Heart of Care will retaliate against you for using this process.
