Consent for Peer Support & Community Services | Heart of Care Full Spectrum CTS

I voluntarily consent to receive peer support and community-based services offered by The Ninety-Nine Retreat, LLC, doing business as Heart of Care Full Spectrum Community Treatment Service ("Heart of Care"). I understand these services are provided by Certified Peer Specialists and trained community workers — not by licensed medical, psychiatric, psychological, or clinical staff. Peer support is grounded in lived experience and the Eight Dimensions of Wellness (Physical, Emotional, Social, Occupational, Intellectual, Financial, Environmental, Spiritual). It is not a substitute for medical, psychiatric, or emergency care. Services may include: one-to-one peer support, wellness planning, coping-skills coaching, resource navigation, transportation assistance, workbook and Academy participation, community events (The Porch, The Barn, Red Bullet outreach, Cluckshack visits), grief support, and safety planning. I understand my participation is voluntary. I can refuse any service, stop at any time, or ask questions at any time without losing access to other supports. I understand a peer specialist cannot prescribe medication, provide a clinical diagnosis, or take the place of a 911 response in an emergency. In a life-threatening crisis I will call 988 or 911.
