Heart of Care respects your privacy. Our full Notice of Privacy Practices explains: • What information we collect about you (contact info, wellness plan, peer support notes, and anything you share). • How and when we may use or share it (coordination of your care with people you name, mandatory reporting when required by law, and de-identified data used for program improvement). • Your rights: to see your record, to ask for corrections, to know who we've shared with, to request restrictions, and to receive communications in a specific way. • How to file a complaint if you feel your privacy has been violated — with us and with the U.S. Department of Health & Human Services.
