Notice of Privacy Practices

This covers information shared with me while I am serving you. Website cookies and shop orders are covered separately in the website privacy policy. Last updated September 2, 2026.

The short version: What you tell me stays with me. I share it only with your written permission, or in the few situations where the law leaves me no choice — and I will tell you when that happens if it is safe to do so.

What I collect

  • Your name, contact information, and how you prefer to be reached.
  • What you tell me about your situation, goals, health, housing, and history.
  • Notes about our visits: dates, times, what we worked on, referrals made.
  • Intake, consent, and release-of-information forms you complete.
  • For personal care work, the care tasks performed and any incident reports required.

I collect the minimum I need to do the work and support a referral. If you would rather something not be written down, say so and we will talk about what that means.

How I use it

  • To serve you, keep track of your goals, and follow up.
  • To coordinate with people you have authorized in writing, and no one else.
  • To meet recordkeeping and billing requirements for the work performed.
  • For grant reporting or program statistics, in aggregate, de-identified form only — never your name or story unless you sign a specific release.

When I must share without your consent

These are the only situations, and they are narrow:

  • A serious, immediate risk that you will harm yourself or someone else, when a warning or emergency contact may prevent it.
  • Suspected abuse, neglect, or exploitation of a child, an elder, or a disabled adult, which Georgia law requires me to report.
  • A valid court order or subpoena.
  • A medical emergency where responders need information to keep you alive.

I will disclose the least amount necessary, and I will tell you what I shared and with whom, unless telling you would put someone in danger.

Your stories and photographs

Nothing about you appears on this website, in a newsletter, on TikTok or Facebook, in a grant application, or in any printed material without a signed release naming exactly what may be used. You may withdraw that release at any time and I will take the material down going forward.

Your rights over your record

  • Ask what I have and get a copy.
  • Ask that an error be corrected, or add your own statement to the file.
  • Ask for a list of what has been disclosed and to whom.
  • Revoke a release of information in writing.
  • Ask me to contact you a specific way, or not to leave voicemail.
  • File a complaint — see the grievance page. Nothing changes about your service.

How it is kept and how long

Paper forms are kept locked at the business address. Digital records are stored in access-restricted systems that only I can open. Records are retained as long as required for the service performed and then destroyed securely. If a breach ever affected your information, you would be told directly and promptly.
